Register all of your children in the school district at one time at the Education Services Center, 5776 South Crocker Street. 

Enroll your child(ren) at Centennial Academy (see map) in the office and fill out the necessary paperwork, at which point you can obtain class assignments, schedules, and other information.

Eligibility: Children who will be 5 years of age by October 1, 2009, are eligible for fall kindergarten 2009.

What you will need:

• Birth Certificate (with state seal)
• Immunization Records showing 5 DTP, 4 Polio, 2 MMR, 3 Hepatitis B, and 2 Varicella (chickenpox)
• Proof of residency (closing papers, rental agreement, or current utility bill with name and address)
